Section A: General Open
Section B: Creative;  Section C - Monochrome.  After sending the Open and/or Creative Section, please return to this form and use Section A for Monochrome BUT name them as C1, C2, C3 and C4 and then submit the Monochrome section.

Please read the Entry Rules  before filling out this form.  You MUST add .jpg:  surname-x-####-yz.jpg  Use only the left side to input your information; the right side describes what should go into the slots on the left.
Please review the Instructions at the bottom of this form.  When you have completed the form, press the Submit button at the end of the page.

INSTRUCTIONS:  To Start, Click inside Field Window T1.  Use "Tab" Key to advance to next Field.
Do not use the ENTER KEY:  Use the "Tab" Key or Mouse Click to advance to the next field. Leave unused fields blank.

Since there are two possible Sections, please insert the appropriate letter A, B, in the Image Filename as shown below:
Use the following Format for your Image Filename:  surname-x-####-yz.jpg
Where x is the first character of your given name.
#### is the last four digits of your phone number.
Where y is the Section Letter (A, B,)
Where z is the image number (1,2,3,4).

Example:  smith-j-1234-A1.jpg      Image Number 1 entered in Section A, by John Smith, phone no. 727-784-1234
Filenames are not case sensitive.
